East Arnhem Regional Council is seeking an experienced and passionate Aged Care & Disability Services Care Coordinator, to deliver culturally safe, person‑centred support to care recipients and people living with disability across our remote communities. This is your opportunity to contribute to the wellbeing, independence, and quality of life of people in one of Australia's most unique and culturally rich regions. What You'll Do
- Coordinate culturally safe assessments, referrals, and co‑designed care plans.
- Lead and mentor service support staff to ensure high‑quality service delivery.
- Work collaboratively with the Regional Manager - Disability Services to ensure participants receive supports in line with NDIS and Aged Care requirements.
- Partner with the Aged Care & Disability Services Operations Coordinator to deliver effective, responsive services.
- Ensure accurate documentation, reporting, and compliance with legislative, funding, and quality standards.
- Support recruitment, rostering, administration tasks, and continuous improvement initiatives.
- Foster strong relationships with clients, families, carers, colleagues, and community stakeholders.
- Promote safe work practices aligned with WHS, risk management, SIRS, and complaints processes.
What You'll Bring Essential
- Cert IV in Individual Support (Ageing and/or Disability), Diploma of Nursing, Community Services, or equivalent experience.
- Current First Aid and CPR.
- Ability to co‑design and manage culturally safe, rights‑based care plans.
- Knowledge of CHSP, Home Care, Support at Home Program, and NDIS guidelines.
- Experience using aged care/disability electronic platforms and government portals.
- Excellent organisational, communication, critical thinking, and stakeholder engagement skills.
- Ability to work autonomously, prioritise competing demands, and adapt to change.
- Proficiency in Microsoft Office.
- Current C Class Driver Licence.
- Ability to obtain and maintain NDIS Worker Screening and WWCC.
Desirable
- Additional related qualifications.
- Food safety training.
- Experience working in remote First Nations communities.
- High level of cross‑cultural capability and awareness of issues affecting First Nations people.
