Employment Type: Permanent Part Time Position Classification: Clinical Nurse Specialist Gde 2 Remuneration: $124,565.90 - $128,661.92 per annum pro rata Hours Per Week: 16 Requisition ID: REQ669315 About The Role Tweed Valley Hospital Renal Outreach Service is seeking a Clinical Nurse Specialist Grade 2 (CNS2) to join the team! Skills The CNS2 Renal Outreach utilises advanced practice nursing skills in assessment, clinical problem solving and care planning. The role is multifaceted including but not limited to: the nursing management of patients with Chronic Kidney Disease (CKD stage 4-5/eGFR <20), including transplant work-up and dialysis access coordination.* providing pre-dialysis treatment options education (HD, PD & RSC).

  • peritoneal dialysis (PD) training and ongoing PD and home haemodialysis (HHD) management.
  • providing on call support and ongoing education to patients and clinicians.
  • follow up and support for patients who have received a kidney transplant.
