About The Role And Where You Will Be Working

You’ll manage your own portfolio of clients, coordinating tailored care services that truly make an impact. Acting as the key link between clients, families, and care teams, you’ll ensure services are seamless, responsive, and centred around each individual’s needs and goals.

While our central office is located in Rivervale, this role is mostly working from home with various travel as needed to our clients north of the river.

We’ll Also Be Asking You To

  • Coordinating personalised home care services that support independence and wellbeing
  • Building strong, trusting relationships with clients, families, and stakeholders
  • Monitoring and adjusting care plans to meet changing needs
  • Managing client budgets and ensuring funding is used effectively
  • Collaborating with internal teams and external providers for integrated care
  • Ensuring compliance, quality standards, and accurate documentation
  • Contributing to continuous improvement and client satisfaction

To Be Considered For This Role, You’ll Need

  • Experience in aged care or home care coordination
  • Strong organisational skills with the ability to manage multiple priorities
  • Excellent communication and relationship-building skills
  • Knowledge of case management, compliance, and funding frameworks
  • A compassionate, person-centred approach
  • Confidence using systems such as Microsoft Office
  • Certificate IV in Ageing Support or equivalent qualification
