New Graduate Social Worker / Care Partner

We are currently recruiting for a New Graduate Social Worker / Care Partner for an excellent not for profit organisation in the Inner West. This organisation supports older people who live in the local community who have a Support at Home package. The role would entail carrying out assessments, understanding needs, managing budgets and assisting the person to live independently. This role would work mainly with older people from CALD backgrounds. About The Role

• Work with older people who have a Support at Home package and live in the community
• Full Time position, with flexible work hours
• Community based position with a little travel involved
• Based out of the Inner West
• Salary of ~$76k plus super and salary packaging
• Training and support would be provided
• We require someone who has completed their Social Work degree and has full work rights in Australia
• We are ideally looking for someone with placement or other experience working in the aged care area

About the organisation This organisation have been supporting people in the community for the past 50 years. They have an excellent reputation. In addition to their work in Aged Care, they also run other events and programs. They have an excellent work culture and supportive management. You would join an existing team and take on a manageable caseload with admin support. Duties

• Conduct initial assessment and prepare necessary care plans for clients.
• Formally review client care plans on an annual basis to ensure compliance to standards of care for quality review
• Monitor, evaluate and review care plans as required by client care needs
• Liaise with and refer to allied health, dementia and continence services and other medical practitioners as required
• Advise rostering team of client needs
• Ensure all services are addressing client needs according to care plan
• Networking with other services and providers to ensure that the service is relevant to the needs of clients

Requirements:

• We are looking for someone who has completed their Social Work Degree - someone with 1+ years of post qualifying experience is also welcome to apply.
• Previous placement or work experience working in the aged care sector - ideally with some case management experience.
• Someone with full work rights in Australia
• Drivers license and car
