Role Description This is a full-time, on-site role for a Care Coordinator based in the Greater Newcastle Area. The Care Coordinator will assist clients from diverse multicultural backgrounds by providing access to relevant services and support. Responsibilities include conducting client intakes and assessments, coordinating care plans, facilitating referrals to community resources, providing ongoing holistic support, and maintaining accurate records. Collaboration with other team members and external organizations to enhance care outcomes is also a critical part of this role. Qualifications Experience in case management, care coordination, or similar roles in community servicesStrong interpersonal and communication skills with the ability to engage with people from diverse cultural backgroundsKnowledge of community resources and services, including those tailored for migrants, refugees, and multicultural communitiesOrganizational and administrative skills, including the ability to maintain accurate documentationProblem-solving abilities and adaptability to address individual client needs effectivelyProficiency in a second language is highly desirableProficiency in relevant software and systems is advantageousTertiary qualification in Social Work, Community Services, or a related fieldCurrent valid driver’s license and a willingness to travel within the service areas
