Communication eXtra is seeking Expressions of Interest from experienced and motivated professionals interested in working within the NDIS sector in roles focused on Support Coordination, participant capacity building and navigating complex service systems.

This opportunity would suit professionals who enjoy working alongside people with disability to help them understand their NDIS plans, identify appropriate supports, build informal and formal support networks and achieve greater choice, control and independence.
The role draws strongly on the skills used across allied health, social work, developmental education and community services, including assessment, advocacy, case coordination, risk identification, capacity building and collaborative problem-solving.
Depending on your qualifications, skills and experience, responsibilities may include:
- Building strong and respectful relationships with participants and their support networks.
- Collaborating with allied health professionals, disability providers, mainstream services, government agencies and community organisations.
- Coordinating implementation of NDIS plans and assisting participants to make effective use of available funding.
- Developing and maintaining participant service networks.
- Identifying barriers preventing participants from achieving their goals and developing practical strategies to address them.
- Identifying safeguarding concerns, service gaps and risks and escalating these appropriately.
- Liaising with NDIS planners, Local Area Coordinators, plan managers and service providers.
- Maintaining accurate, professional and timely case notes and participant records.
- Monitoring participant budgets, service utilisation and progress towards goals.
- Obtaining reports and supporting evidence required for NDIS reassessments, plan reviews and change-of-circumstances requests.
- Supporting participants to understand their NDIS plans, funding categories and available service options.
- Supporting participants through periods of transition, service breakdown or changing circumstances.
- Working collaboratively with participants to increase their confidence and capacity to independently navigate services wherever possible.

Experience within the NDIS is highly regarded; however, professionals with strong transferable experience from disability, allied health, health, mental health, community services or human services are encouraged to express their interest.
- A relevant qualification in Allied Health, Community Services, Developmental Education, Disability, Human Services, Social Work or a related discipline.
- An understanding of person-centred and strengths-based practice.
- Excellent written communication and report-writing skills.
- Strong organisational and time-management skills.
- Confidence communicating with participants, families, providers and professionals.
- Strong problem-solving and critical-thinking skills.
- The ability to manage competing priorities and work independently.
- An understanding of professional boundaries, confidentiality, dignity of risk and duty of care.
- Experience identifying and responding to safeguarding concerns and participant risk.
- Knowledge of the NDIS and contemporary disability practice, or the ability and willingness to develop this knowledge quickly.
Experience in one or more of the following areas will be particularly valued:

Support Coordination is more than arranging services. The role requires someone who can see the broader picture, identify what is preventing a participant's plan from working effectively and bring together the right people, supports and evidence to create meaningful change. You will often work across multiple systems and stakeholders and may support participants experiencing complex barriers, changing support needs or difficulty accessing appropriate services.
