  • Permanant Part time role based in the Hobart Region
  • Flexible days- 3 days per week (must include Tue & Wed), no weekends.

What You'll Do?

  • Engage in complex and rewarding work by supporting clients to maintain or regain independence, empowering them to stay in their own homes.
  • Provide individualized care, working one-on-one with clients to achieve meaningful clinical outcomes.
  • Collaborate with our multidisciplinary team to deliver the best holistic care, ensuring that clients receive comprehensive support tailored to their needs.
  • Plan and manage your own flexible schedule, working both from home and visiting clients to deliver personalized, impactful care.
  • Contribute to wellness programs, providing health education and helping to design initiatives that promote healthier living in the community.
