Please note that this vacancy may be withdrawn at any time should it be filled via the internal redeployment processJob OverviewPLEASE NOTE THAT WE WILL ONLY ACCEPT APPLICATIONS FROM STAFF CURRENTLY EMPLOYED BY ANEURIN BEVAN UNIVERSITY HEALTH BOARD The post holder will be responsible for delivering a person-centred, multi-layered therapeutic service to older people living with a mental illness in a care setting. The post holder will need to ensure their delivery is cost-effective, responsive, robust, and clinically sound. The post holder will attend care homes to assess, review and offer specialist advice for older people living in a care setting, with a dementia, cognitive impairment, or functional mental illness. The post holder will be required to adapt their skills to suit the specific person’s complex needs, advocating on behalf of the patient and, where appropriate, supporting the least restrictive solutions. To provide complex, sensitive information to patients in a care home setting who have varying degrees of barriers to understanding (mental Illness, dementia or cognitive impairment. Utilising empathy, persuasion and reassurance. Plans and oversees the provision of care for patients in a care home setting. Main duties of the job The patient could be under any part of the mental health measure and therefore, the skills of the nurse will need to be broad and multi-faceted. The post holder will give support, advise, education and assistance to staff working in care homes, within their allocated geographic area, and ensuring the guidance shared is evidence-based. The post holder will utilise other professional groups and/or services to ensure all opportunities are afforded to the patent, to ensure they are well cared for and living as well as possible within the confines of their mental illness. The post holder will support the Directorate to deliver the Welsh Government/ Public Health Wales targets – including reduction in anti-psychotic prescribing. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply. - NMC qualification
