Nurse Manager - Home Services

Your role at St John of God Healthcare at Home We are seeking a dynamic, highly motivated and enthusiastic Nurse Manager to provide leadership and oversight of our service in Geelong. This is an opportunity to lead a service with significant growth and a thriving new home maternity service, working collaboratively with our hospital and external providers to deliver a first-class service. You will provide management of the service as well as deliver direct patient care, as required, to ensure a positive patient experience and clinical outcomes in line with the Home Services Strategy. We are offering a permanent position working 45 hours per fortnight. The Position:

• Leads and fosters a culture of continuous improvement where all patient encounters influence a memorable patient experience, and which is underpinned by excellence and safety in care.
• Monitors, analyses, and reports variances and trends in clinical data to continuously improve clinical quality and safety outcomes.
• Contributes to policy and procedure development to optimise clinical outcomes and enhance the patient experience.
• Actively promotes and integrates service models with the hospitals to deliver seamless care in the community and ambulatory setting.
• Promotes the delivery of evidence-based practice, including research and quality activities to improve patients’ care in the home setting.
• Participates in, contributes to and implements quality improvement and risk management into all aspects of service.

Successful applicants will be registered as a Registered Nurse with the Nursing & Midwifery Board of Australia (AHPRA), hold a Tertiary qualification in nursing, management and/or relevant discipline, and have recent acute clinical experience in home nursing. This position will also require a State drivers license, Working with Children Check and National Police Clearence. • Salary: $127,128 per annum (pro-rata for part-time) plus 12% Superannuation
• Permanent part-time, working 45 hours per fortnight
