Role - Intake & Relationships Specialist Location - Penrith (Hybrid role) Permanent Full Time Join our team and be the guiding light for families in need of care. In this exciting role, you will directly impact our client experience, becoming the face of 365 Care bringing us to life in our local community! Working as part of our Aged Care team, this is an exciting role where you'll developing new client relationships. You will be the first point of contact for our future clients and their loved ones, providing compassionate, and personalised advice relating to their Home Care Package. You’ll be part of a dedicated team that’s committed to ethical, person-centred care. Given the community engagement in this role, regular travel across Western Sydney will be required. What You'll Do

  • Follow up on all new client enquiries, nurturing each client’s journey working closely with them from their first contact until they are ready for a face-to-face meeting with one of our Care Partners
  • Provide tailored advice offering comprehensive information and practical guidance on their Home Care Package
  • Uphold person-centred ethics, approaching every interaction with integrity ensuring each client’s experience is aligned with their best interests and personal needs
  • Identify and attend relevant Community Engagement initiatives acting as our representative in the community

What We're Looking For

  • A natural people person who loves a great conversation
  • Knowledge of the aged care sector, Support at Home/Home Care Package experience is a bonus
  • Experience leading the end to end intake process with heart
  • Active listening skills and the ability to adapt your approach to each client's unique situation
  • Ability to thrive in a dynamic, agile environment
